FieldsResearch offers a simple flat fee per month with a small one-time setup fee. There are no 'cost per survey' or 'cost per patient' charges that many vendors charge in addition to large initial contract fees. You don't have to 'trade-up' or choose a more expensive 'package' to see your data in a useful way - we provide free on-line and off-line viewing.

Pricing and Payment Terms


FieldsResearch is an approved vendor, and we will conduct your surveys for as low as $95 a month depending on the size of your agency plus a one-time $200 setup. CMS’s target is 300 completed surveys per year. If your number of decedents does not allow for that quantity, we will charge a lower monthly fee based on your average monthly decedents. There is no additional cost for Spanish.There is a reason why we have an A+ BBB rating.

 

You pay a simple flat rate (shown above) and a one-time setup fee of $200. That’s it... NO other charges, period. After the setup is paid, you will be charged on a quarterly basis. Payment is processed at the beginning of the first month of each quarter (January 1st - covering October, November & December decedents, April 1st - covering November, December & January decedents, July 1st - covering April, May & June decedents, and October 1st - covering July, August & September decedents). If you would like to have FieldsResearch handle your compliance, call Ken today at 513-821-6266.

Why Phone Mode

FieldsResearch will utilize telephone only mode. Telephone produces a higher response rate than mail only and is more cost effective than a mixed methodology. Telephone provides more complete and reliable data, as it is automated by computer, but administered by well-trained telephone research interviewers, resulting in no skipped questions and fewer 'don't know' responses. It is our opinion that disgruntled patients will fill out a mail survey at a higher rate than content ones. We provide bilingual interviewers at no additional cost to handle calls to your Spanish-speaking patients.

CAHPS Hospice Survey Overview

The CAHPS Hospice Experience of Care Survey assesses the experience of patients who received hospice care, as well as the experiences of their informal primary caregivers. Because the CAHPS Hospice Survey focuses on experiences of care, implementation of the survey supports the following national priorities for improving care: involving patients and families in care and promoting effective communication and coordination.

The standardized 47 question CAHPS Hospice Survey instrument is composed of the following measures:

  • Hospice Team Communication
  • Getting Timely Care
  • Treating Family Member with Respect
  • Providing Emotional Support
  • Support for Religious and Spiritual Beliefs
  • Getting Help for Symptoms
  • Information Continuity
  • Understanding the Side Effects of Pain Medication
  • Getting Hospice Care Training

CMS Requirements

The Hospice Quality Reporting Program (HQRP) was mandated by Section 3004 of the Affordable Care Act and requires all Medicare-certified hospices to submit quality data to CMS. This requirement has been linked to the CAHPS Hospice Survey for the Annual Payment Update (APU) for FY2017. All hospices will be required to conduct a “dry run” of the survey for at least one month in January, February, or March 2015. As of April 2015, hospices are required to participate in the CAHPS Hospice Survey program on a continuous monthly basis. Those who fail to do so will receive a 2% reduction in their 2017 APU.

For 2016, hospices are required to submit 12 months of survey data to the CAHPS Hospice Survey Data Warehouse. CY 2016 participation will affect the 2018 APU.

CAHPS Hospice Exemption

There are only two exemptions from participation:

  • If the hospice served fewer than 50 deceased patients in the calendar year (CY) preceding the exemption request
    • Hospices must reapply for exemption on a yearly basis
    • Hospices must submit a Participation Exemption for Size Form here to apply for exemption
  • If the hospice received its Medicare Provider Number/ CCN on or after the first day of the performance year for the CAHPS Hospice Survey
    • One-time exemption
    • Hospices will be notified by CMS if they are exempt

*Very small hospices: Hospices that have fewer than 50 deceased patients in the period from January 1, 2015 through December 31, 2015 will be exempt from the Hospice Experience of Care Survey data collection and reporting requirements for the FY 2017 payment determination. The due date for submitting an exemption request form for the CY 2017 APU was August 12, 2015.

Hospice Responsibilities

As your contracted vendor, FieldsResearch would be responsible for administering the CAHPS Survey on a monthly basis and submitting the data to the CAHPS Hospice Survey Data Warehouse.

The CAHPS Hospice Survey is currently a “pay-for-reporting” program, which means that the submission of required data by specified deadlines, rather than performance, determines compliance. Data submission requirements include the following:

  • Providers must contract with a CMS-approved survey vendor who will collect and submit the data on the provider’s behalf
  • Providers are required to participate in a Dry Run for at least one month in the first quarter of 2015 (January or February or March)
  • Providers are required to participate on a continuous monthly basis starting April 2015
  • This means that for 2015, providers are required to submit a minimum of 10 months of data to the CAHPS Hospice Survey Data Warehouse. This includes one month during the Dry Run and monthly for all nine months April through December 2015. (Providers who participated for more than one month during the Dry Run may submit all months to the CAHPS Hospice Survey Data Warehouse.)
  • In subsequent years continuous monthly participation is required
  • Providers that fail to meet the CAHPS Hospice Survey requirements as specified in rulemaking will receive a 2 percentage point reduction in their APU
  • The participation requirements for CY 2015 will affect the 2017 APU. Participation requirements for subsequent years will impact subsequent APUs.
