Our process and price are simple and straightforward. We are a family owned and operated business. We are one of the longest standing data collection firms in the United States with over 40 years of conducting surveys. We specialize in data collection - we don't have the added expense of extra layers of personnel that the 'full service' consulting companies have, requiring them to pass on additional overhead costs, even to clients who simply want to comply with the CMS requirement.

FieldsResearch offers a simple flat fee per month with a small one-time setup fee. There are no 'cost per survey' or 'cost per patient' charges that many vendors charge in addition to large initial contract fees. You don't have to 'trade-up' or choose a more expensive 'package' to see your data in a useful way - we provide free on-line and off-line viewing.

Pricing and Payment Terms

CMS’s target is 300 completed surveys per year. If your patient census (defined as a patient with a payer source of either Medicare and/or Medicaid) does not allow for that quantity, we will charge a lower monthly fee based on your average monthly census...

There is no additional cost for Spanish.

You pay a simple flat rate (shown above) and a one-time setup fee of $200. That’s it... NO other charges, period. After the setup is paid, you will be charged on a quarterly basis. Payment is processed at the beginning of the second month of each quarter (February, May, August and November). If you would like to have FieldsResearch handle your compliance, sign up today.

Why Phone Mode

FieldsResearch will utilize telephone only mode. Telephone generally produces a higher response rate than mail only, and is more cost effective than a mixed methodology. Telephone provides more complete and reliable data, as it is automated by computer but administered by well-trained telephone research interviewers, resulting in no skipped questions and fewer 'don't know' responses. It is our opinion that disgruntled patients will fill out a mail survey at a higher rate than content ones.

We provide bilingual interviewers at no additional cost to handle calls to your Spanish-speaking patients.

About the Survey

The Home Health Agencies CAHPS (HH‑CAHPS) Survey gauges the experience of patients receiving home health care. According to CMS, the three goals of the HH‑CAHPS survey are to improve quality of care, enhance public accountability, and produce standardized data to allow for meaningful comparisons between home health agencies.

The survey consists of 25 “core” and 9 “about you” questions that produce four quality reporting measures of patient experience encompassing:

  • Patient care (gentleness, courtesy, problems with care).
  • Communication with health care providers and agency staff about care
  • Specific care issues related to pain and medication
  • Overall rating of care

Patients considered eligible for inclusion in the HH‑CAHPS Survey:

  • Medicare/ Medicaid patients
  • Patients served in all branches that fall under your CCN number
  • Patients at least 18 years of age at any time of their stay and believed to be alive
  • Patients that have received at least two visits in the past 60 days and at least one skilled care visit during the sample month
  • Patients not currently receiving only hospice care
  • Patients not currently receiving only routine maternity care
  • Patients who have not requested "no publicity" status
  • Patients not excluded due to state survey restrictions

CMS Requirements

HH‑CAHPS has been linked to the quality reporting requirement for the Annual Payment Update (APU). To obtain the full APU, all Medicare-certified Home Health Agencies (HHAs) that annually serve 60 or more survey-eligible patients are required to contract with an approved HH‑CAHPS Survey vendor to administer the survey on a monthly basis and submit HH‑CAHPS Survey data to the HH‑CAHPS Data Center on a quarterly basis.

HH‑CAHPS Exemption

To apply for an exemption for the CY 2018 APU period, HHAs must count and report to CMS the number of survey-eligible patients served during the APU Reference Period via the online HH‑CAHPS Survey Participation Exemption Request Form, available here.

2020 APU Reference Period: between April 1, 2018 and March 31, 2019
2021 APU Reference Period: between April 1, 2019 and March 31, 2020

There are only two exemptions from participation:

  • If an HHA received Medicare certification after the cutoff date for the APU Reference Period, it is ineligible to participate in the upcoming APU
    • HHAs do not have to apply for this exemption
    • Cutoff date for CY 2020 APU: April 1
  • If an HHA served fewer than 60 survey-eligible patients during the APU period, it may apply for an exemption
    • The HHA must submit a Participation Exemption Request (PER) form online for the APU Reference Period for which they are requesting exemption
    • HHAs must submit a PER form every year they seek exemption
    • PER form deadline for CY 2020 APU: March 31

Agency Responsibilities

As your contracted vendor, FieldsResearch would be responsible for administering the HH‑CAHPS Survey on a monthly basis and submitting data to the HH‑CAHPS Data Center on a quarterly basis.

The responsibilities of an HHA participating in HH‑CAHPS are to:

  • Register for login credentials at https://homehealthcahps.org/
  • Contract with approved HH‑CAHPS survey vendor and authorize to collect and submit survey data on HHA's behalf to HH‑CAHPS Survey Data Center
  • Work with vendor to set date each month by which monthly patient information file for sampling and fielding HH‑CAHPS survey will be provided
  • By said date, compile and deliver complete and accurate patient information file
  • Ensure vendor has submitted data on time and without problems by reviewing survey data submission reports on https://homehealthcahps.org/
    Review results prior to public reporting
  • Avoid influencing patients about how to answer survey
  • Understand APU and how it relates to HH‑CAHPS Survey
